The four doses

Each strength is a separate tablet, and you move up the ladder one step at a time. The intervals between steps are set to give your body time to adjust — the exact timing for you is decided by your prescriber based on how you are tolerating each stage, not by a rigid calendar.

  • 4mg — titration: the first step up, building the dose as early side effects settle
  • 9mg — titration: a further increase toward a therapeutic level; some patients find their comfortable maintenance point here

A point worth underlining: the numbers on the tablet look large next to the injection’s (which tops out at 2.4mg weekly), but this reflects how much has to be given to account for the smaller fraction absorbed orally — not that the tablet is a stronger treatment. Why you start at the lowest

Starting at 1.5mg is not about caution for its own sake — it is the main tool for managing side effects. The gastrointestinal effects of semaglutide are most pronounced when the body first meets a new dose, so a low starting point and gradual increases give your digestive system time to adjust at each stage. Jumping straight to a high dose would not speed up weight loss; it would front-load the nausea and, for many people, end treatment before it got going. The slow ladder is what makes the medicine tolerable enough to stay on — and staying on it is what produces results.

How escalation decisions are made

Moving up a dose is a decision made at review, not a foregone conclusion when a set number of weeks passes. If the current dose is comfortable and working, moving up may still make sense to build toward your target. If side effects are still settling, the right move is to hold — give the current dose more time before adding to it. The schedule serves you; you do not serve the schedule.

The aim is the lowest dose that achieves your goals comfortably — and “comfortably” is not a soft word here, because comfort is what keeps you on treatment long enough to benefit. If you are doing well at 9mg, staying there is a legitimate and often sensible outcome, reviewed over time as your needs change. How long does it take to reach the 25mg dose?

It is a gradual climb over several months, with the exact pace set by how you tolerate each step rather than a fixed timetable. Rushing it tends to backfire through side effects. Some patients settle at a lower dose and never need to reach 25mg.

Do I have to reach the highest dose to lose weight?

No. Many patients see good progress at 9mg or even lower. The best dose is the one that balances results with how well you tolerate it — not automatically the top of the ladder. That is a judgement made at your reviews.

What if a dose increase makes me feel unwell?

Tell us. Often the answer is to hold the current dose longer, or step back down, before trying again. A short repeat of the early side effects after an increase is common and usually settles; if it does not, we adjust the plan. Our side effects guide has more.

Why are the tablet doses so much higher than the injection’s?

Because only a fraction of an oral dose is absorbed, so more has to be given to reach effective levels. The milligram number reflects the delivery route, not the strength of treatment. The 25mg tablet and the 2.4mg injection are both full maintenance doses of the same medicine.

Can I go back down a dose if I need to?

Yes — stepping back down is a normal part of managing treatment, not a failure. If a higher dose is not sitting well, dropping to the previous one is often the right move. The ladder works in both directions.
