Why the HPV vaccine matters more than most people realise

HPV — Human Papillomavirus — is one of the most common infections in the world. Most people who contract it never know, because it rarely causes symptoms and the immune system usually clears it on its own. The problem is that in some people it does not clear, and a persistent infection with a high-risk strain can, over years, cause cell changes that develop into cancer. HPV is responsible for 99.8% of all cervical cancers, and for a significant proportion of anal, penile, throat, and mouth cancers too.

Around 3,200 women are diagnosed with cervical cancer in the UK each year, and around 880 die from it. These are preventable deaths. The HPV vaccine is one of the most effective tools we have to reduce them, and the evidence from the UK vaccination programme is now extraordinary. Cervical cancer has been almost eliminated in the youngest vaccinated cohorts.

The vaccine cannot treat an existing HPV infection. What it does is train your immune system to recognise and fight the virus before exposure occurs, or before you have been exposed to all the strains it covers. The earlier you are vaccinated the better, but for most adults there is still real benefit to being vaccinated now.

This matters because the original school vaccination programme used an earlier version of the vaccine covering only four strains. Gardasil 9 covers five additional high-risk strains on top of those.

In practical terms, Gardasil 9 protects against HPV types responsible for approximately 90% of all cervical cancers and 90% of all genital warts. If you received the original Gardasil vaccine at school, you can still benefit from Gardasil 9. It will extend your protection to cover the five additional high-risk strains.

Who should consider getting the HPV vaccine privately in Edinburgh?

Adults aged 25 to 45 who were never vaccinated. A significant proportion of people in this age group were not covered by the school programme and have never been vaccinated against HPV. If you have not been exposed to all nine strains covered by Gardasil 9, vaccination still offers meaningful protection against cancer and genital warts.

Anyone who received the original 4-strain Gardasil at school. You can receive Gardasil 9 in addition to completing the original course. This extends your protection to cover five additional high-risk strains not included in the vaccine you received at school.

Men who want protection. HPV causes anal cancer, penile cancer, and throat and mouth cancers in men as well as women. Older men were not included in the school programme extended to boys in 2019. Private vaccination is the only route to protection for men aged 25 and over who were not vaccinated as teenagers.

Men who have sex with men (MSM). MSM face a significantly higher risk from HPV-related anal cancer. Free vaccination is available at sexual health clinics up to age 45, but appointments can be difficult to access quickly. Edinburgh Vaccination Clinic offers a straightforward private alternative.

The vaccine does not replace cervical screening

Gardasil 9 does not protect against every strain of HPV that can cause cervical cancer. Women should continue to attend cervical screening (smear tests) from age 25 regardless of vaccination status. Vaccination and screening work best together.

How many doses will you need?

The number of doses depends on your age at the time of your first vaccination. Our pharmacist will confirm the right schedule for your circumstances at your first appointment.

People under 25 typically require one dose. People aged 25 and over typically require two doses, given at least six months apart. Three doses may be recommended for those who are immunocompromised. Doses are given as a single injection into the upper arm. Your pharmacist will book your follow-up appointment before you leave if more than one dose is needed.

What to expect at your appointment

Your appointment starts with a short consultation. Our pharmacist will ask about your age, any previous HPV vaccination, your medical history, and any medications or allergies. This takes around ten minutes and ensures the vaccine is appropriate for you. If everything is in order, you receive your first dose the same day.

The vaccine is given as an injection into the upper arm. Most people experience no significant side effects. The most common reactions are mild soreness or redness at the injection site, and occasionally a slight headache or low-grade temperature that resolves within a day or two. Serious reactions are very rare. We ask that you remain at the clinic for a short observation period after each dose as a standard precaution.

If you require more than one dose, your follow-up appointment will be booked before you leave. The whole process is straightforward, the consultation is confidential, and the entire first appointment including vaccination typically takes around 20 to 30 minutes.

What does getting vaccinated actually change?

Since the HPV vaccination programme began in the UK, rates of cervical cancer in vaccinated cohorts have fallen dramatically. A landmark study published in The Lancet found that girls vaccinated at age 12 to 13 saw cervical cancer rates reduced by 87%. That is not a marginal improvement. For many people vaccinated at the right age, it removes the risk of cervical cancer almost entirely.

Rates of genital warts in young vaccinated populations have also fallen sharply in countries with high vaccination uptake. For men, the benefits extend to protection against anal cancer, penile cancer, and throat and mouth cancers, conditions linked to HPV that are entirely preventable through vaccination.

NHS England has set a formal target to eliminate cervical cancer by 2040. That ambition is built on the vaccine working. Getting vaccinated is not just about protecting yourself now. It is part of one of the most significant public health achievements of a generation.

No. Gardasil 9 is licensed for use up to age 45. If you have not previously been exposed to all nine strains covered by the vaccine, vaccination still offers meaningful protection. I had the HPV vaccine at school. Should I get Gardasil 9?
