What shingles actually does, and why it matters at 50

If you had chickenpox as a child, the varicella-zoster virus never fully left your body. It retreated into the nerve tissue around your spinal cord and brain and has been dormant ever since. In most people it stays that way. But as the immune system weakens with age, the virus can reactivate. When it does, it causes shingles.

Shingles typically begins with a burning or stabbing pain on one side of the body, often before any rash appears. Within a few days a blistering rash develops, usually across the chest, abdomen, or face. The rash lasts two to four weeks in most cases. The pain during an active episode is frequently described as one of the most intense experiences a person has ever had. For many people it is debilitating: difficult to sleep, difficult to work, difficult to function normally.

The rash is only part of the problem. In up to 1 in 5 people who develop shingles, the nerve pain does not stop when the rash heals. This is called post-herpetic neuralgia (PHN) and it can persist for months or even years. For older adults in particular, PHN can be genuinely life-changing. The vaccine does not just reduce your risk of getting shingles. It significantly reduces your risk of the pain that follows.

Shingrix: the vaccine we use at Bruntsfield Pharmacy

At Edinburgh Vaccination Clinic (Bruntsfield Pharmacy), we use Shingrix — the recombinant, non-live shingles vaccine that replaced the older Zostavax in the UK vaccination programme in 2023. Zostavax has now been discontinued in the UK entirely, having expired from the programme in November 2024. Any clinic or pharmacy still offering Zostavax is out of date. Shingrix is the only shingles vaccine currently in use in the UK.

The reason Shingrix replaced Zostavax is straightforward: it is dramatically more effective. Clinical trial data show Shingrix is over 90% effective at preventing shingles and post-herpetic neuralgia in adults aged 50 and over. Long-term follow-up data published in 2024 confirmed that efficacy remains at 79.7% in adults aged 50 and over, six to eleven years after vaccination. That is a level of sustained protection that Zostavax could not match.

Shingrix is also a non-live vaccine, which is clinically significant. Because it contains no live virus, it can be used in people with weakened immune systems for whom a live vaccine would have been unsuitable. This includes people on immunosuppressive medications, those undergoing cancer treatment, and people with certain chronic conditions.

Important: Zostavax has been discontinued in the UK

The older Zostavax shingles vaccine was withdrawn from the UK programme in September 2023 and the last batch expired in November 2024. It is no longer available. Shingrix is now the only shingles vaccine used in the UK. If you were previously vaccinated with Zostavax, speak to your pharmacist about whether Shingrix vaccination is appropriate for you.

How effective is Shingrix?

The clinical trial results for Shingrix are among the strongest seen for any adult vaccine. In the pivotal ZOE-50 and ZOE-70 trials, Shingrix demonstrated over 90% efficacy against shingles in adults aged 50 and over. Crucially, this efficacy held across all age groups tested, including adults in their seventies and eighties, where older vaccines typically saw a significant drop-off in effectiveness.

Long-term follow-up data presented at a major infectious diseases conference in 2024 confirmed that Shingrix maintains strong protection more than a decade after vaccination, with 79.7% efficacy in adults aged 50 and over at the six to eleven year mark. Boosters are not currently recommended, though this will continue to be reviewed as the data matures.

Who should consider the Shingrix vaccine?

Shingrix is licensed for adults aged 50 and over, and for adults aged 18 and over who are immunocompromised. The following groups have particular reason to consider private vaccination at Edinburgh Vaccination Clinic (Bruntsfield Pharmacy)

Immunocompromised adults aged 18 and over. Shingrix is non-live, it is safe for use in most people with weakened immune systems. Your pharmacist will assess suitability at your consultation.

People who have previously had shingles. Having had shingles once does not guarantee you will not get it again. Vaccination can reduce the risk of a further episode. Shingrix can be given at any time after recovery, provided the acute infection has resolved and there are no active blisters. Your pharmacist will confirm suitability and timing at your consultation.

Adults previously vaccinated with Zostavax. Zostavax offered significantly lower protection than Shingrix. Current NHS guidance does not routinely recommend re-vaccination with Shingrix for those who have already completed a Zostavax course. However, if you are unsure about your vaccination history or have specific health circumstances, our pharmacist can review your situation and advise whether Shingrix is appropriate for you.

Who should not have Shingrix

Shingrix is not suitable for everyone. It should not be given to anyone who has had a severe allergic reaction to a previous dose or to any ingredient of the vaccine. It should be postponed if you are currently unwell with a fever of 38.5 degrees or higher. It is not recommended during pregnancy.

The Shingrix course: what to expect

Shingrix is given as a two-dose course. For immunocompetent adults in Scotland, the second dose is given between two and six months after the first. For immunocompromised adults, the second dose can be given as early as eight weeks after the first, with the second dose given eight weeks to six months after the first.

Both doses are needed for full protection. Your pharmacist will book your follow-up appointment before you leave after your first dose. The vaccine is given as an injection into the upper arm. Most people experience no significant side effects beyond mild soreness or redness at the injection site. Some people experience muscle aches, headache, or a brief flu-like feeling for a day or two. This is a normal immune response and resolves quickly.

What to expect at your appointment

Your appointment begins with a short consultation. Your pharmacist will ask about your age, your medical history, any current medications, any previous shingles episodes, and whether you have been vaccinated against shingles before. This typically takes around ten minutes and confirms whether Shingrix is appropriate for you.

If everything is in order, you receive your first dose the same day. The whole first appointment including vaccination typically takes around 15 minutes.
