Wegovy Pill vs Rybelsus: Same Drug, Different Licence

The Wegovy pill and Rybelsus contain exactly the same active ingredient: oral semaglutide, made by the same manufacturer. So the question patients keep asking is a fair one — are they the same medicine, and if so, why does it matter which one you are prescribed? The answer sits in a distinction that the online sellers rarely explain properly: the difference between a molecule and a licence. The molecule is identical. The licence — what the medicine has been approved for, at what doses, on the strength of which trials — is not, and the licence is what protects you.

One molecule, two licences

Rybelsus has been available in the UK since 2020 as a treatment for type 2 diabetes. Its licence was granted on the strength of trials measuring blood glucose control in people with diabetes. The Wegovy pill was licensed by the MHRA in June 2026 for weight management, on the strength of trials — the OASIS programme — measuring weight loss in people with obesity or weight-related conditions. Same molecule; entirely different questions asked of it, answered in different patient groups, at different doses.

A licence is not paperwork for its own sake. It defines the doses shown to work for a specific purpose, the monitoring that goes with them, and the safety data collected in the relevant population. When a prescriber stays within the licence, every part of your treatment — the escalation schedule, the review points, the safety thresholds — is anchored to evidence gathered for exactly what the medicine is being used to do. Step outside the licence and that anchor is gone.

The dose difference and why it matters

The practical gap between the two products is at the top of the dose ladder. The Wegovy pill escalates through 1.5mg, 4mg and 9mg to a 25mg daily maintenance dose — the strength studied in the weight management trials, and the strength behind the published results. Rybelsus is licensed only at lower strengths, because glucose control in diabetes is achieved at lower doses than meaningful weight loss.

This is why “Rybelsus for weight loss” was always a compromise. Before June 2026 there was no licensed oral option, and some providers supplied Rybelsus off-licence to patients who wanted a tablet. Whatever you think of that trade-off then, it is obsolete now: a tablet designed, dosed and licensed for weight management exists. Choosing the off-licence product today means accepting lower doses than the evidence supports, outside the licence, usually to save money. That is not a trade we will make with your treatment.

Why we don’t prescribe Rybelsus for weight loss

Our position is straightforward: where a licensed option exists for the condition being treated, that is what we prescribe. Off-licence prescribing has a legitimate place in medicine — but “the unlicensed version is cheaper” has never been a good clinical reason, and it is a particularly poor one when the licensed product is a tablet of the very same molecule. Prescribing the Wegovy pill means your dose ladder matches the weight management evidence, your reviews are benchmarked against the right trial outcomes, and there is no ambiguity about what you are taking or why.

There is a second reason, and it is about supply. Rybelsus exists for people with type 2 diabetes. When it is diverted at scale to weight loss, the people who depend on it for glucose control face shortages — the same pattern that played out with Ozempic. Prescribing the product licensed for the purpose is also the responsible thing to do for other patients.

If you’ve been offered Rybelsus online

If an online provider has offered you Rybelsus for weight loss — or you are taking it for that purpose now — you have not done anything wrong, and neither necessarily has the prescriber. But you are entitled to ask why you are on an off-licence product when a licensed one exists, and what the plan is for your dose. If the honest answer is price, you deserve to make that trade-off knowingly rather than by default.
