What matters is that the eligibility assessment is a clinical conversation, not an automated screening tool — and that if you are unsure, a consultation is the right next step rather than a reason to delay.

The clinical eligibility criteria for Mounjaro
Mounjaro (tirzepatide) is licensed in the UK for chronic weight management in adults. The MHRA-approved eligibility criteria mirror those used in the SURMOUNT clinical trials and set a clear threshold based on BMI and the presence of weight-related health conditions.
You are likely eligible for Mounjaro if:
- Your BMI is 30 kg/m² or above (obesity range), or
- Your BMI is 27 kg/m² or above and you have at least one of the following weight-related conditions: type 2 diabetes, high blood pressure (hypertension), high cholesterol (dyslipidaemia), obstructive sleep apnoea, or cardiovascular disease
These criteria apply to adults aged 18 and over. You must not be pregnant, planning to become pregnant, or breastfeeding.
If you are unsure of your BMI, your pharmacist will calculate it at the consultation. A BMI of 27 is the point at which excess weight is considered to begin carrying meaningful clinical risk when accompanied by a related health condition — which is why the threshold is lower for patients in this group.
Weight-related conditions that can lower the eligibility threshold
For patients with a BMI between 27 and 29.9 — technically classified as overweight rather than obese — Mounjaro is still available if at least one of the following conditions is present. These are conditions where excess weight is known to be a contributing factor and where weight loss is likely to produce meaningful clinical benefit beyond the number on the scales.
Type 2 diabetes
Mounjaro has a dual mechanism relevant to type 2 diabetes — it improves insulin sensitivity and reduces blood glucose independently of its weight loss effect. In SURMOUNT-2, patients with type 2 diabetes lost an average of 15.7% of their body weight at the 15mg dose, with significant improvements in HbA1c. For patients on diabetes medication, your prescriber will review potential interactions and may advise on dose adjustments to existing treatments.
High blood pressure (hypertension)
Excess weight is one of the most significant modifiable risk factors for hypertension. Weight loss through Mounjaro has been shown in clinical trials to reduce systolic blood pressure meaningfully — in some cases to the point where patients are able to reduce antihypertensive medication under clinical supervision.
High cholesterol (dyslipidaemia)
Mounjaro has demonstrated favourable effects on lipid profiles in clinical trials, including reductions in LDL cholesterol and triglycerides. Patients with elevated cholesterol levels alongside overweight often see meaningful improvements in their lipid markers alongside weight loss.
Obstructive sleep apnoea
Excess weight around the neck and upper airway is a primary driver of obstructive sleep apnoea. Substantial weight loss — of the kind Mounjaro produces — can significantly reduce the severity of sleep apnoea and in some cases resolve it altogether, reducing reliance on CPAP therapy.
Cardiovascular disease
Patients with established cardiovascular disease and a BMI of 27 or above are eligible for Mounjaro. The SURMOUNT-CVOT trial demonstrated a statistically significant reduction in major adverse cardiovascular events in this patient group, making Mounjaro one of the few weight loss medications with direct cardiovascular outcome data.
Polycystic ovary syndrome (PCOS)
PCOS is not listed as a named qualifying condition in the Mounjaro licence, but it is a recognised weight-related condition associated with insulin resistance, and patients with PCOS who also have hypertension, dyslipidaemia, or type 2 diabetes will meet the eligibility criteria on those grounds. Your pharmacist will review your full clinical history at the consultation.

Who is not suitable for Mounjaro?
There are specific contraindications to Mounjaro that your pharmacist will review at the consultation. If any of these apply, Mounjaro will not be prescribed — and your pharmacist will discuss alternatives including Wegovy (semaglutide), where appropriate.
Mounjaro is not suitable if you have:
- A personal or family history of medullary thyroid carcinoma (MTC)
- Multiple Endocrine Neoplasia syndrome type 2 (MEN2)
- A history of acute pancreatitis or chronic pancreatitis
- Pregnancy, breastfeeding, or planned pregnancy
- A known allergy to tirzepatide or any component of the Mounjaro injection
- Type 1 diabetes (Mounjaro is not licensed for type 1 diabetes)
If you have type 2 diabetes and are taking insulin or a sulphonylurea, you may still be eligible — but your prescriber will need to review your current medications carefully, as dose adjustments may be necessary to avoid hypoglycaemia.
What the eligibility assessment involves
The assessment at Bruntsfield Pharmacy is conducted by a qualified Independent Prescriber pharmacist. It is thorough, unhurried, and designed to give you a clear answer — either a prescription and your first pen, or an honest explanation of why Mounjaro is not the right option and what is available instead.
- Pre-appointment questionnaire — completed online before your visit, covering weight history, medical conditions, and current medications.
- In-person review — your pharmacist confirms your BMI, reviews your health history, checks for contraindications and relevant drug interactions, and answers your questions.
- Prescription and supply — if Mounjaro is clinically appropriate, your private prescription is issued and your first pen dispensed at the same appointment. Injection training is included.
- Ongoing monitoring — dose escalation reviews at each 4-week step up, with access to the clinical team between appointments.

What BMI do I need to get Mounjaro?

A BMI of 30 or above makes you eligible without any additional conditions. A BMI of 27 or above makes you eligible if you also have at least one weight-related condition such as type 2 diabetes, hypertension, high cholesterol, sleep apnoea, or cardiovascular disease.
Can I get Mounjaro if my BMI is under 27?

Mounjaro is not licensed for use below a BMI of 27. Prescribing it below this threshold would be outside the MHRA-approved licence and is not something we would do at Bruntsfield Pharmacy. If your BMI is below 27 and you are concerned about your weight, your pharmacist can discuss other evidence-based options.
Does PCOS qualify me for Mounjaro?

PCOS itself is not a named qualifying condition in the Mounjaro licence. However, many patients with PCOS also have type 2 diabetes, insulin resistance diagnosed at the point of hypertension or dyslipidaemia, or other conditions that do qualify. Your pharmacist will review your full history to determine eligibility.
I have type 2 diabetes and take insulin — can I still get Mounjaro?

Possibly, but it requires careful clinical review. Mounjaro can cause significant reductions in blood glucose, which may increase the risk of hypoglycaemia in patients taking insulin or sulphonylureas. Your prescriber will review your current diabetes medications and may recommend adjustments before starting Mounjaro. This is a conversation worth having at consultation rather than a reason to assume you cannot proceed.
What if I am not eligible for Mounjaro?

If Mounjaro is not appropriate for you, your pharmacist will explain why and discuss alternatives. Wegovy (semaglutide 2.4mg) is available at Bruntsfield Pharmacy and has a similar eligibility profile. There may also be other clinical options worth discussing depending on your specific situation.
Do I need a GP to confirm my eligibility before booking?

No. Our Independent Prescriber pharmacists conduct the full eligibility assessment independently. You do not need a GP referral, a letter, or any documentation from your GP before attending. If your GP has relevant medical records you would like to share, you are welcome to bring them — but they are not required.
