What is Shingrix and why does it matter?

Shingrix is a recombinant, non-live vaccine that protects against shingles and the nerve pain that can follow it. It replaced the older Zostavax in the UK vaccination programme in September 2023, and Zostavax expired from circulation entirely in November 2024. It is no longer available anywhere. Shingrix is now the only shingles vaccine used in the UK.

The switch happened because Shingrix is dramatically more effective. In the pivotal ZOE-50 and ZOE-70 clinical trials, it demonstrated over 90% efficacy against shingles in adults aged 50 and over. That figure held across all age groups tested, including people in their seventies and eighties, where older vaccines typically lost effectiveness sharply. Long-term follow-up data published in 2024 confirmed efficacy of 79.7% at the six to eleven year mark after vaccination.

Because Shingrix contains no live virus, it can be used in people with weakened immune systems who could not safely receive a live vaccine. This includes people on immunosuppressive medications, those undergoing or recovering from cancer treatment, and adults with certain chronic health conditions.

Around 1 in 4 people will develop shingles at some point in their lives, and the majority of cases occur in people over 50. The older you are when shingles strikes, the more severe the episode typically is, and the higher the risk of post-herpetic neuralgia, the nerve pain that can persist long after the rash has healed.

One of Shingrix’s most remarkable qualities is that its efficacy does not decline with age. Clinical trial data confirm no clinically meaningful differences in protection across age groups from 50 into the eighties and beyond. This is precisely what makes it so different from Zostavax, which lost effectiveness sharply with age. With Shingrix, getting vaccinated at 50 delivers the same level of protection as getting vaccinated at 70. Protection lasts more than a decade based on current trial data, which means vaccinating in your fifties gives you meaningful coverage well into your sixties and beyond.

What Shingrix protects you against

Most people know shingles as a painful rash. Fewer people know that the rash is often the least of the problem. The real danger is what comes after: post-herpetic neuralgia (PHN), a form of nerve pain that can persist for months or years once the rash has cleared. Up to 1 in 5 people who develop shingles go on to experience PHN. In older adults, it can be severe enough to affect sleep, work, and daily function for a prolonged period.

Shingrix reduces the risk of shingles by over 90% and reduces the risk of PHN by a similar margin. Even in the rare cases where someone vaccinated with Shingrix does develop shingles, the evidence suggests the episode is milder and shorter than it would otherwise have been. In Scotland, around 400 people aged 70 and over are hospitalised due to shingles every year. Vaccination significantly reduces this risk.

  • Shingles itself — the painful blistering rash caused by reactivation of the varicella-zoster virus
  • Post-herpetic neuralgia (PHN) — nerve pain that can persist for months or years after the rash
  • Ophthalmic shingles — the form affecting the eye, which carries a risk of permanent vision loss
  • Shingles-related hospitalisation — in Scotland, around 400 people aged 70 and over are admitted each year

Current guidance does not routinely recommend re-vaccination for those who completed a full Zostavax course, but if you are unsure about your vaccination history your pharmacist can review your circumstances and advise.
