Can adults over 60 get the yellow fever vaccine in Edinburgh?
Yes, in some cases, after a careful individual risk assessment. Adults aged 60 and over are not automatically excluded from yellow fever vaccination. What changes at this age is the depth of the clinical assessment we complete before any vaccine is given. The aim of that assessment is to weigh up the clinical risk of vaccination against the real-world risk of yellow fever exposure on your trip, then arrive at a recommendation that is appropriate for you. In some cases that means proceeding with Stamaril and issuing your International Certificate of Vaccination or Prophylaxis (ICVP). In other cases it means recommending against vaccination and discussing alternative ways to manage your risk, including a Medical Letter of Exemption where appropriate.
Why the age 60 threshold matters clinically
Yellow fever vaccination is generally well tolerated. Most people who receive Stamaril experience either no reaction or a mild, short-lived response: soreness at the injection site, a mild headache, muscle aches, or a low-grade temperature for a few days. Serious adverse reactions to the vaccine are rare.
However, two rare but recognised serious adverse events, sometimes referred to as YEL-AVD (yellow fever vaccine-associated viscerotropic disease) and YEL-AND (yellow fever vaccine-associated neurologic disease), are reported at a higher rate in adults aged 60 and over, particularly in those receiving the vaccine for the first time. The absolute risk remains low, but the relative increase is the reason UK and international travel health guidance recommends a more detailed suitability check in this age group.
For adults aged 60 and over, vaccination is usually only recommended where there is a significant and unavoidable risk of yellow fever exposure. The clinical conversation in over-60s is therefore not really about whether the vaccine works (it does) but about whether the risk profile is acceptable for your specific trip. That is why we ask more questions, and review the destination and itinerary more carefully, before we vaccinate.
Putting the risk in context
The absolute risk is low. Serious adverse events from yellow fever vaccination are rare in any age group.
The relative risk is higher in over-60s. UK and international travel health guidance recognises a higher reporting rate of YEL-AVD and YEL-AND in adults aged 60 and over, particularly in first-time vaccine recipients.
Yellow fever itself is a serious disease. A real exposure in a yellow fever risk area can also have serious consequences. The clinical assessment weighs both sides.
How we assess suitability in adults over 60
If you book a yellow fever appointment with us and you are aged 60 or over, expect the consultation to be a little longer and more detailed. The pharmacist will work through three connected questions: how much yellow fever exposure your trip is likely to involve, whether there are any clinical reasons to be cautious about vaccinating, and whether the certificate is being sought primarily for medical protection, for border requirements, or for both.
What we review at the consultation
- Your destination and itinerary, including whether you are travelling to high-risk rural or jungle areas or staying in lower-risk urban areas
- Whether you have had the yellow fever vaccine before, since first-time recipients in this age group are reviewed more carefully
- Your medical history, with particular attention to immune system status
- A history of thymus disease, including thymoma or myasthenia gravis, or previous thymectomy, which are recognised contraindications
- Current medications, including any treatment that affects the immune system
- A history of severe egg allergy or known allergy to any vaccine ingredient
- The country requirements at your destination, including whether an ICVP is needed for entry or transit
The outcome of this assessment is a clear recommendation, with the reasoning explained. If vaccination is appropriate, the dose is given at the same visit and your ICVP is issued. If it is not appropriate, we discuss the alternatives below.

If the vaccine is not suitable: alternatives for over-60 travellers
For some over-60 travellers, the clinical recommendation is not to vaccinate. The most common reasons are a recognised contraindication (such as a thymus disorder, significant immunosuppression, or a relevant allergy history) or a risk-benefit balance where the trip itself carries very low yellow fever risk and the vaccine risk in your age group is judged too high to justify routine vaccination.
When that is the outcome, there are practical alternatives to discuss at the consultation.
Alternatives if vaccination is not appropriate
- Medical Letter of Exemption. If yellow fever vaccination is contraindicated and your destination requires proof of vaccination for entry, we can issue a Medical Letter of Exemption as a designated Yellow Fever Vaccination Centre. Acceptance of this letter is ultimately at the discretion of the receiving country, so we will discuss this realistically with you.
- Itinerary review. In some cases, an adjustment to your itinerary, such as avoiding the highest-risk rural or jungle areas, materially reduces your exposure to yellow fever and changes the conversation about what is needed.
- Strict mosquito-bite prevention. Yellow fever is spread by mosquitoes. For travellers who are not being vaccinated, careful day and evening bite avoidance is the foundation of risk reduction.

When should I book my yellow fever appointment if I am over 60?
Timing is more important for over-60 travellers than it sometimes is at younger ages, because there is a real possibility the consultation outcome is a Medical Letter of Exemption rather than vaccination. Both pathways take time to plan, and an exemption letter is most useful when there is room to discuss it with the airline or destination authorities in advance if needed.
In practice, we recommend booking your appointment at least four to six weeks before travel where possible. This window allows:
- Time for the ICVP validity period (the certificate becomes valid 10 days after vaccination)
- Time to review the rest of your travel vaccine needs in one consultation
- Flexibility to plan around a Medical Letter of Exemption if that becomes the outcome

I had the yellow fever vaccine years ago. Do I need another dose now I am over 60?
Under the 2016 World Health Organization International Health Regulations update, a single dose of yellow fever vaccine is considered to provide lifelong protection for the purposes of the ICVP, and a booster dose is generally not required. Whether you would benefit from a further dose in specific circumstances is something we review at the consultation, alongside your destination and medical history.
How long should the appointment take?
Plan for a longer appointment than for a routine travel vaccine.
